Ciguatera in the Colombian Caribbean: history and Characteristics 2010-2014

Introduction: ciguatera is a disease caused by the consumption of some fish living in tropical and subtropical regions that contain toxins produced by dinoflagellates; these enter the food chain, with humans being the final stage, causing the disease. Every year, between 25 000 and 500 000 people are affected worldwide, causing an impact on health and the economy.
Objective: to conduct an analysis of the disease in the Colombian Caribbean, describe cases, outbreaks (two or more related cases) and determine the incidence between 2010 and 2014 in San Andres and Providencia.
Materials y methods: a systematic literature review of ciguatera cases identified in the country and a descriptive study of cases and outbreaks reported to the Public Health Surveillance System for San Andres and Providencia during 2010 to 2014 were carried out.
Results: according to the literature, more than 80 cases of ciguatera were reported in the Colombian Caribbean between 1968 and 2007, with no deaths. Between 2010 to 2014, 101 cases and 22 outbreaks were reported from San Andres and Providencia, with no associated mortalities and an incidence of 17,4 per 100 000 inhabitants. The main associated fish species were barracuda and jack mackerel; mainly gastrointestinal and neurological signs and symptoms were reported.
Conclusions: cases of ciguatera reported in the public health surveillance system were identified. The incidence in San Andres and Providencia is similar to that of the Cayman Islands and exceeds that of other Caribbean countries such as Cuba, Jamaica, Belize and Bermuda, among others. The fish species identified are consistent with the main species involved in other countries. It is recommended to train territorial health entities on seaweed toxin intoxications for the Caribbean zone in order to strengthen the reporting of the event and the quality of the data.
