Background. In Colombia, on May 23, 2023, a positive Mpox case was reported to the Public Health Surveillance System—Sivigila— corresponding to a patient previously confirmed on September 22, 2022. An immediate response team was deployed to investigate the situation. The objective was to study a probable case of reactivation, reinfection, or prolonged Mpox. Methods. Case report study based on outbreak investigation methodology by the CDC. Information sources included the epidemiological notification form, medical records, expanded field epidemiological investigation, laboratory results, and genomic characterization. A descriptive analysis of data was presented. Results. A 25-year-old man with Mpox lasting nine months, concurrently affected by HIV-AIDS, syphilis, and oropharyngeal candidiasis, with superinfection of ulcerated eosinophilic pustular folliculitis caused by multisensitive and persistent Escherichia coli and Staphylococcus aureus. Genomic characterization [U2.1][U2.2] indicated persistent infection. Conclusion. Prolonged Mpox infection was confirmed, worsened by immunosuppression due to untreated HIV- AIDS. Reinfections, reactivations, or prolonged Mpox infections are rare and should be studied by healthcare personnel and public health surveillance teams in Colombia.References
