Analysis of the Estimated Reporting Rate of Leukemias and Timeliness Indicators in Children Under 15 by Region, Colombia 2022-2023

Abstract

Introduction. Childhood cancer is the second leading cause of death in children under 15. Acute lymphoblastic leukemia is the most common hematologic cancer; it mainly affects children aged 2 to 4 and accounts for 23 % of diagnoses in various regions worldwide. Objective. To estimate the reporting rate of leukemias and timeliness indicators in children under 15, based on records in the Public Health Surveillance System (Sivigila) during the 2022-2023 period. Methodology. This is a retrospective descriptive observational study by epidemiological year 2022 and 2023. An analysis was conducted of the estimated reporting rate by territorial entity (TE) of residence and of timeliness indicators for newly confirmed leukemia cases in children under 15, according to the event protocol Code 115 of the National Institute of Health (INS). Results. Between 2022 and 2023, 991 new leukemia cases were reported in Colombia, with rates of 4,3 and 4,1 per 100 000 inhabitants under 15 years old. The departments with the highest rates in 2022 were: Arauca, Cartagena de Indias D.T., Guaviare, Meta, Nariño, Risaralda, Archipiélago de San Andrés y Providencia, Tolima and Vichada, y en 2023 fueron: Boyacá, Caquetá, Chocó, Guainía, Guaviare, Norte de Santander, Santander, Santa Marta D.T. y Vichada.While some regions showed good opportunities for presumptive diagnosis, variability was evident in diagnostic confirmation times and treatment initiation, with significantly lower timeliness especially in areas with possible limitations in specialized pediatric oncology services. Conclusion. Some regions of Colombia report high leukemia notification rates and delays in timely care, hindering access to comprehensive diagnosis and treatment for children and adolescents under 15.
